Aging Life Care™ Professionals

A Care Manager, also known as an Aging Life Care™ Professional, is a health and human services specialist who acts as a guide and advocate for families who are caring for older relatives or disabled adults. The Aging Life Care Professional is educated and experienced in many fields including, but not limited to nursing, gerontology, social work, and psychology with a specialized focus on issues related to aging and elder care.

Our ultimate goal is to improve the quality of life for our clients while bringing peace of mind to their families.

  • Comprehensive Care Planning – We develop personalized short-term and long-term care plans that evolve as needs change, helping families make informed decisions with confidence.
  • Expert Care Coordination – We manage services across multiple healthcare providers and agencies, ensuring seamless transitions, reducing administrative burdens, and improving continuity of care.
  • Objective Assessments and Home Visits – Through comprehensive in-home evaluations, we identify safety concerns, recommend evidence-based interventions, and monitor changing care needs.
  • Building the Right Care Team – We help families identify, vet, and coordinate qualified caregivers, home care providers, and medical professionals who best meet their loved one's needs.
  • Cost Containment – By preventing unnecessary hospitalizations, avoiding duplication of services, and helping families make appropriate care decisions, we protect both financial resources and quality of care.
  • Facilitating Difficult Conversations – As experienced, compassionate professionals, we guide families through challenging discussions such as driving cessation, living arrangements, advance care planning, and end-of-life decisions.

How do you know if you need a Care Manager / Aging Life Care™ Professional?

Caring for an aging or vulnerable loved one can be both rewarding and challenging. As care needs become more complex, partnering with a Care Manager / Aging Life Care™ Professional can provide expert guidance, advocacy, and peace of mind for both you and your family.

You may benefit from the support of a Care Manager / Aging Life Care™ Professional if your loved one:
  • Is no longer safe living independently but is resistant or hesitant to accepting help or making necessary changes.
  • Has a complex medical condition, such as Dementia, Alzheimer's disease, or Parkinson's disease, and you need education, guidance, and strategies to manage changing care needs and behaviors.
  • Requires professional advocacy to coordinate care, communicate effectively with healthcare providers, navigate the healthcare system, or ensure their needs and wishes are being addressed.
  • Is experiencing confusion about financial or legal matters, making them vulnerable or unable to manage important decisions independently.
  • Has limited or no local family support, leaving caregiving responsibilities to one person or creating gaps in care.
  • Has caregiving needs that have become overwhelming, leaving family members feeling stressed, uncertain, or in need of professional guidance to develop a safe, effective care plan. A Care Manager serves as your family's trusted partner—helping assess needs, coordinate services, advocate for quality care, and provide ongoing support so your loved one can maintain the highest possible quality of life while giving family caregivers confidence and peace of mind.
