Skilled Nursing Services are prescribed by a physician to help with rehabilitation after an illness, injury, hospital stay, or surgery, or to help manage a chronic medical condition with the goal of preventing an unnecessary hospitalization. This care is provided through one-hour home visits from specialized clinicians and continues until recovery goals have been met.
Registered Nurses and Licensed Practical Nurses will provide direct care as well as teach you and your caregivers on how to manage your care.
Speech, Occupational, Respiratory and Physical Therapists will help individuals maximize their physical and mental well-being.
Medical Social Workers will assess an individual’s situation and help to identify resources in the community that can provide assistance.
